This is form 1 of 3. This form collects basic information: name, email, and decision. If the decision is not yes, they receive 1 follow up question. If the decision is need more info, they receive 1 follow up question and then move to form 2, where they answer a limited number of questions. If the decision is yes, they move directly to form 2. Campaign memberships are created for all respondents at this step.

This form has a prefill connector that looks up the Opportunity. Based on the opportunity, the form will conditionally display instructions to enter a school-provided email address. Otherwise, users can enter a person or work email (or both) as they please.

This form is sent to schools/districts that have purchased seats and will need to recruit educators to fill those seats. This form will also work for ESDs.
